The SmartChart included in Answers is your one stop shop for a fullview of your residents’ Electronic Health Records. You and your staff will be able to toggle between all of the clinical areas in Answers to quickly and efficiently reach the information you are searching for without navigating menus. You will also enjoy the tight integration of information between all of the clinical areas that will assist your staff in having the latest information across departments and disciplines all from one integrated chart on-screen. Staff can be instantly alerted to important changes regarding the residents under their care through the embedded Alerts & Messaging function.
Everything you Can Do on Paper & More
Answers EHR will be your staff’s first line of defense to finding, entering and tracking information about your residents. It includes a complete physician’s orders component with full eMAR and eTAR capabilities which enhance your clinicians’ ability to accurately track med-pass assignments by resident, shift, floor and more. You will also be able to easily track infections, incidents, falls, wound care, TB Screenings, immunizations, vital signs and weights. Your staff will have the flexibility to enter ID notes for each resident from various screens. Pre-populated templates are available to help your staff reduce the time it takes for proper documentation, while still providing the necessary information for the resident’s records.
Electronic signatures ensure that accountability and security remains within your operations. These signatures allow for the signing of everything from physician’s orders to ID notes. Electronic signatures are password protected and can only be entered or removed by their authenticated owner.
If your organization really wants to get rid of paper, Answers offers an attachment function which allows your staff to securely save scanned files to each resident’s profile. Retrieval is easy due to the organization of your files by logical tabs and user-defined types.
Freedom to Work Your Way
Answers is designed to be flexible and allow each organization the freedom to personalize their system to their particular needs. This flexibility flows through the entire software solution and is a core benefit for your organization. In the clinical side, a user defined forms capability allows your organization to create and maintain comprehensive assessment forms that you can adjust to your facility’s specific needs. Although it is pre-built with 14 assessments, any number of additional assessments can be entered to define the categories and formats that work best for you. You also have the ability to update the MDS and resident EHR from these assessments. You can even set your own scores for each assessment question in order to come up with your own outcomes benchmark.
Answers can automatically generate Care Plans based on the RAP/Trigger for your residents. This will save time and ensure that all triggered RAPs are identified and addressed in accordance with sound clinical practices and provides for a systematic flow of information between the MDS, Care Plans, and Point of Care collection tools.
Completing the Circle of Care
Answers EHR also includes a new Point of Care solution for documentation and monitoring of ADLs, Care Plan items, vitals and more. This system is available to CNAs via a simple to use touchscreen and also offers bird’s eye views of daily charting activities for supervisors and DONs. It automatically updates the MDS and is integrated as part of the full EHR for each Resident.
A Great Tool for MDS Coordinators
Answers also provides an intuitive and comprehensive MDS format that can be autopopulated from our Point of Care application to track ADLs and from many other areas of the system. There are also interfaces to other popular Point of Care tools. MDS Coordinators will transition to the Answers MDS system easily because it looks and feels just like a paper MDS.

- RUG scores can be re-calculated at any time and the system assists you in determining what your ARD should be to maximize your billing potential.
- All of the information from the MDS is integrated to the Billing module for processing.

- Answers MDS also automatically schedules both OBRA and PPS assessments for you.
